Registering with the OSD

New Undergraduate, Graduate, and Professional School students who have been admitted to UC San Diego and who have “Accepted the Offer” should contact the OSD immediately to schedule an intake appointment. Currently enrolled students should register with the OSD whenever a disabling condition arises. Students enrolled in Extension courses need to contact Annie DeLoach at 858.822.1366 or unex-ssd@ucsd.edu.

If you are Deaf or Hard of Hearing, please email Alejandra at deaf-hohrequest@ucsd.edu to request an appointment with a Disability Specialist. To expedite scheduling, indicate the days/times (Monday-Friday, 8:00am-3:00pm) in the subsequent two weeks when you are available. Also indicate if you would like an interpreter or captionist to be present at your appointment.

 

Currently Enrolled or Newly Admitted Freshman/Transfer Students who HAVE “Accepted the Offer” to attend UC San Diego

 

The first step in the registration process is to contact the OSD at 858.534.4382 and schedule a New Student Intake Appointment with a Disability Specialist. This appointment may be conducted in person or over the phone. Please let our staff know your preference when you schedule.

At or before the first meeting, students should complete the Intake and Request for Accommodations Form and Consent Form. To assist the Disability Specialist in preparing for your meeting, please return these forms ahead of time via fax (858.534.4650) or email (osd@ucsd.edu).

Students will also need to provide documentation of their disability, and the Office has created forms to assist medical professionals with providing appropriate information. Documentation Forms may be found here.  It is very important that the documentation address your current, functional limitations, particularly as they pertain to a higher education academic setting. This documentation can be faxed to 858.534.4650 or emailed to osd@ucsd.edu before your appointment.

 

Newly Admitted Freshman\Transfer Students who HAVE NOT yet “Accepted the Offer” to attend UC San Diego

 

If you are a newly admitted freshman or transfer student seeking information about accommodations, please call the OSD at 858.534.4382 to schedule a Prospective Student Intake Appointment. This intake appointment may be conducted in person or over the phone. Please let our staff know your preference when you schedule.

At or before the first meeting, students should complete the Consent Form and either the Prospective Freshman Intake Form or Prospective Transfer Intake Form. To assist the Disability Specialist in preparing for your meeting, please return these forms ahead of time via fax (858.534.4650) or email (osd@ucsd.edu).
