Registering with the OSD

New undergraduate, graduate, and Professional School students who have been admitted to UC San Diego and who have submitted their intent to register should contact the OSD immediately to discuss accommodation needs. Currently enrolled students should register with the OSD whenever a disabling condition arises. Students enrolled in Extension courses need to contact Annie Dinh at (858) 822-1366 or ssd@ucsd.edu .

The first step in the registration process is to contact the OSD and schedule a phone or in-person appointment with a Disability Specialist. At or before the first meeting, students should complete the OSD Intake Form and OSD Consent Form. Students will also need to provide documentation of their disability, and the Office has created forms to assist medical professionals with providing appropriate information. Documentation Forms may be found here. If you are uncertain as to which Documentation Form should be completed, please ask. It is very important that the documentation address your current, functional limitations particularly as they pertain to an academic setting.
